The mission of palliative and end-of-life care is to meet the physical, emotional and spiritual needs of patients and their loved ones. Now, a local institution has been honored with a Circle of Life Award for its innovation in palliative care and for improving the care of patients near the end of life. It is being held up as a model for others in the field.

BY TRANSFORMING OUR CULTURE AND INVESTING IN PALLIATIVE CARE FOR PATIENTS AND FAMILIES FACING SERIOUS, OFTEN LIFE-LIMITING ILLNESSES, WE HAVE CREATED A SWEEPING CONTINUUM OF CARE THAT INCLUDES SPECIALTY SUPPORTIVE AND PALLIATIVE TRAINING, TEAMS AND CLINICS, AS WELL AS A HOME-BASED ADVANCE ILLNESS MANAGEMENT PROGRAM. THIS SYSTEM-WIDE APPROACH HAS IMPROVED PATIENT OUTCOMES AND THE OVERALL CARE EXPERIENCE.
