(Richmond, VA, Wednesday, July 13, 2016) - The American Hospital Associationâ€™s Circle of Life Awards honor innovative palliative and end-of-life care in hospices, hospitals, health care systems, long-term care facilities, and other direct care providers. Three programs that expand the reach of palliative and end-of-life care will be honored with the 2016 Circle of Life Award: Celebrating Innovation in Palliative and End-of-Life Care, along with three programs awarded a Citation of Honor. The award, now in its 17th year, celebrates programs across the nation that have made great strides in palliative and end-of-life care.
This year, Bon Secours Palliative Medicine will receive a Circle of Life award.

OUR PALLIATIVE MEDICINE PROGRAM FOLLOWS PATIENTS AND THEIR FAMILIES WHEREVER THEY RECEIVE TREATMENT, INCLUDING HOSPITALS, OTHER SPECIALTY PRACTICES AND THEIR OWN HOMES. AS PART OF THE MEDICAL GROUP, WE USE PRIMARY AND SPECIALTY PALLIATIVE CARE AS CORNERSTONES OF THE PROGRAM AND ARE INVOLVED IN INNOVATIVE DELIVERY AND PAYMENT MODELS.
OUR INTERDISCIPLINARY TEAMS FOCUS STRONG ATTENTION ON THE PSYCHOLOGICAL AND SOCIAL NEEDS OF PATIENTS AND FAMILIES, AS WELL AS STAFF. WE ARE ALSO INVOLVED WITH THE COMMUNITY AND TWO OTHER LOCAL HEALTH SYSTEMS IN EMBEDDING ADVANCE CARE PLANNING DISCUSSIONS INTO PRIMARY CARE PRACTICES.
